Is my payment receipt acceptable for verifying healthcare expenses?
We need to see the service date, amount, and services rendered for your medical services to be reimbursed. Documents showing only the payment date and provider name aren’t sufficient because a payment date doesn’t always fall on the date the service was rendered. Purchase receipts also don’t generally include details about the services you received.

Are there any circumstances where a payment receipt can be used?
We can use a payment receipt if “copay” is typed on it. Below are a few examples.

What documents can I submit instead of a payment receipt?
Other documents you could submit to verify your expenses include an explanation of benefits (EOB) from your insurance provider or an itemized invoice/statement from the service provider.
